Who can BPCC share my information with?
Your doctor may discuss your information with other professionals to assist in providing service to you. If this is with someone outside of BPCC, you will be asked to give permission. You have the right to refuse this.

You are required to give your verbal and/or written consent before any information about your health care is shared with other agencies except in exceptional circumstances that require the service to breach confidentiality.  

Please be aware that in order to provide you with the best possible care, your doctor may consult with other health care professionals and that auditing of patient records will occur from time to time. 

How can I  change my personal information?
You may phone or come in to the clinic to change your personal details. It is important we have your correct address and contact number if we ever need to get in touch with you.

How is my personal information protected?
BPCC is bound by law to protect personal and health information from misuse or loss, unauthorised access, modification,disclosure or destruction. This applies to all of your information, whether computer or paper based. All computers in BPCC are password protected.

Results:
All patient results will be reviewed by a doctor as they recieve them. If all results are normal and no further discussion is required we will not contact you for a further appointment. 

If a doctor or nurse requires to see you for a follow -up appointment you will be contacted by either nurse or a receptionist who will organise an appointment for you based on urgency of results.

If you can not be contacted after 3 attempts we will send you a registered letter in the mail for you to give reception a call and arrange an appointment time to suit yourself and the doctor.

Comments and concerns:
Comments and concerns are welcome at all times. If you have a comment of concern you can:
- Fill out a patient feedback form
- Speak directly to the Chief Executive Officer, or
- Speak directly to your doctor

Your comment or concern will be dealt with as soon as possible. Please leave a contact name and number for us to contact you, if required.
